COVID-19 Statement

This statement contains important information about our decision (yours and mine) to conduct in-person services in light of the COVID-19 public health crisis. Please read this carefully and let me know if you have any questions.

Decision to Meet Face-to-Face

Due to the many unknown factors involving the transmissibility and severity of coronavirus, it is impossible for me to ensure your absolute safety. You are responsible for assessing your own risk factors and comfort level regarding meeting in-person

You understand that by coming to the office, you are assuming the risk of exposure to the coronavirus and other public health risks.

Your Responsibility to Minimize Your Exposure

To obtain services in-person, you agree to take certain precautions which will help keep everyone (you, me, our families, and other clients) safer from exposure. 

  • You will only keep your in-person appointment if you are symptom free. Symptoms include but are not limited to: fever, fatigue, dry cough, loss of appetite, shortness of breath, mucus/phlegm, sore throat, headache, chills, loss of smell or taste, stuffy nose, nausea/vomiting, diarrhea.

  • You will arrive no earlier than five minutes before your appointment time, and will maintain a minimum six foot distance from others while in the waiting room.

  • You will wash your hands or use alcohol-based hand sanitizer upon entering the building, waiting room, or office.

  • We will keep a distance of 6 feet and there will be no physical contact (e.g. no shaking hands) during sessions.

  • You may wear a mask and/or request that I wear one, as well. (You must supply your own mask.)

  • If someone in your home tests positive for the infection, you will immediately let me know so that we can transition to telehealth sessions until the quarantine period has passed.

I may change the above precautions if additional local, state or federal orders or guidelines are published.

My Commitment to Minimize Your Exposure

I have taken steps to reduce the risk of spreading the coronavirus within the suite and my office building. These efforts include supplying hand sanitizer in the office and thorough cleaning at regular intervals.

If You or I Are Sick

If you have symptoms listed above, please let me know at your earliest convenience so that we can transition to telehealth. Or you may reschedule your appointment without penalty/late cancel fees.

If I am sick, I will immediately cancel all in-person sessions and notify you as soon as possible.
